The module takes place at the cardiothoracic unit at Castle Hill Hospital, Cottingham, Hull.
The Cardiothoracic Unit is purpose-built unit comprising of two dedicated wards which have 51 beds for cardiothoracic surgery and 11 beds used as cardiology beds dedicated to the work of a purpose-built Cardiac Catheterisation Laboratory; and a 9-bedded Intensive Care Unit, of which two beds are dedicated for general admissions.
The Unit has four cardiothoracic surgeons, who between them do 800 open heart procedures yearly, 300 major thoracotomies, 700 endoscopies and 200 minor thoracic procedures which include mediastinoscopies and thoracospies.
In the purpose-built Cath Lab three cardiologists perform, on a regular basis, several procedures including coronary angiographies, coronary angioplasties, insertion of trans-vascular pacemakers, electrophysiology studies and insertion of internal defibrillators. Many of these procedures require anaesthetic cover and input.
The anaesthetic cover for the Cardiothoracic Unit comprises of eight consultant anaesthetists and five sub-consultant grades which are one Associate Specialist, one Staff Grade, two SpR's and currently one SHO which will be replaced by a FTTA registrar in the future. The sub-consultant resident rota is therefore 1:5 shared between the five sub-consultant grades.
The Trainees during this module are trained in administering anaesthesia for cardiothoracic procedures and share in looking after patients in the Intensive Care Unit, both general and cardiothoracic; rotated through to the Cath Lab to learn about anaesthetic support for Cath Lab procedures and also participate in the Acute Pain Service within the Cardiothoracic Unit, which involves placement and management of narcotic thoracic epidurals, paravertebral and PCA iv narcotics.
The Trainees share in all educational and audit activities of the Department, both on the Cardiothoracic Unit level and East Coast School of Anaesthesia level. The trainees also have the opportunity to engage in research.
