Intensive Care Training Module

ICU' are located at:

• Hull Royal Infirmary - one general and one neurosurgical
• Castle Hill Hospital - 7 cardiac and 2/3 general beds
• Grimsby
• Scunthorpe

The Hull Royal Infirmary (HRI) ICU is a 10 bedded general ICU (currently staffing 8 beds). It admits critically ill patients from all specialities except cardiac and burns. The ICU manages both paediatric and neurosurgical patients and there is also a separate 5 bedded neurosurgical ICU/HDU which is imminently due for expansion and relocation. Trainees will also gain experience in cardiac ICU whilst on the cardiac block at Castle Hill Hospital. Rotating trainees will have the opportunity to work in both Grimsby and Scunthorpe ICU's.

The majority of ICU training takes place in the Hull Royal Infirmary unit. It is covered 24 hours a day by 7 consultants who perform daily business and teaching ward rounds and provide a high level of supervision and teaching for trainees in ICU.

ICU training is comprised of:

• Basic - Three months at SHO level some of which may be in Scunthorpe or Grimsby - this is intended to provide a comprehensive introduction to general Intensive Care.
• Intermediate - Every 4th or 5th day on ICU for a 24 hour period at SPR 1/2 level. During this period there is a great deal of opportunity to gain experience in all aspects of the management of critically ill patients with close consultant supervision. Trainees have the opportunity to learn a broad spectrum of practical procedures including arterial, central and pulmonary arterial line insertion and percutaneous tracheostomy. Informal teaching on ward rounds etc. occurs regularly and when time allows the consultants often hold more formal tutorials on the ICU.
• Regular ICU tutorials are held monthly with an emphasis on new developments and likely topics for the final FRCA examination. These are held in the early evening out of the hospital followed by a take away and a few beers - they have proved very popular!
• SPR 4 trainees can opt to spend a block in ICU for further experience.

ICU training nationally is currently in the early stages of redevelopment and formalisation, guided by the Intercollegiate Board for Training in Intensive Care Medicine.
